Have ready: your balances by year, your proposed monthly amount and the date you want it drafted, your bank routing and account numbers for direct debit, and if the balance is above the streamlined limit, the completed 433-F.
YOU: Good morning. I'm calling about my account for tax year[s] [years]. I'd like to set up an installment agreement. [Streamlined:] My total balance is $[amount], all my returns are filed, and I'm proposing $[amount] per month by direct debit on the [day] of each month, which pays the balance within 72 months. [Non-streamlined:] My total balance is $[amount]. I have a completed Form 433-F and I'm proposing $[amount] per month, which is my disposable income under the standards and pays the balance in full before the collection statute expiration dates.
[Identity verification. For non-streamlined, the income and expense walk-through from F-1.]
REP: [Quotes a higher payment.]
YOU: Can you tell me which expense you adjusted and why? [Listen. If an expense was capped at the standard:] I'm proposing to pay within six years, so under the six-year rule I'd ask that my actual [housing / vehicle] cost be allowed. [If the IRS says the plan runs past the CSED:] Can you tell me the expiration dates you're using for each year? [Then, if needed:] I can increase the payment to $[amount] beginning [date], when [a loan ends / other]. Can the agreement be written with a step-up?
REP: [Approves.]
YOU: Thank you. A few confirmations. The first draft date is [date] for $[amount]. The late-payment penalty rate drops to a quarter percent now that the agreement is approved, correct? Will a Notice of Federal Tax Lien be filed? [If yes and you qualify to avoid it:] My balance is under the streamlined limit and I'm paying by direct debit; I'd ask that no lien be filed. [If a levy is in place:] Since the agreement is approved, I'm requesting release of the levy on [account/employer] under section 6343(a)(1)(C). Can that be faxed to [number] today?
YOU, before hanging up: To confirm: the agreement is for $[amount] per month by direct debit on the [day], beginning [date], [stepping up to $[amount] on [date]], covering tax years [years]. [The levy release is being faxed to [number].] I'll receive written confirmation by mail. May I have your name and ID number?
