Use this model letter

Copy or download the blank model letter, then replace every [bracketed field] and remove alternatives that do not apply. Read the guidance below before using it. These are templates, not official IRS forms; nothing is filed or sent for you.

Downloads work without JavaScript. Copy letter becomes available when the page is interactive.

Word is editable; PDF is for reading or printing. No account or personal information is needed.

When to use this

Use this model when you intend to end a representative's authority to deal with the IRS for you and want to document that decision clearly. First identify the authorization and the tax matters and periods you intend to revoke.

Gather the Form 2848 if you have it, the representative's name and CAF number if known, your current contact information and details of any replacement representative. Read the current Form 2848 instructions for the revocation procedure and where to send it; do not assume that ending a private engagement automatically updates IRS records.

This letter addresses representation, not the underlying tax balance or a response deadline. Keep a signed copy and proof of delivery, and record follow-up in your IRS communication log.

Model letter and instructions

Use when ending a relationship with a representative.

I revoke the Form 2848, Power of Attorney, executed on [date] in favor of [representative name], CAF number [if known], for all tax matters and periods listed on that form. A copy of the form is enclosed with "REVOKE" written across the top and my signature and today's date below.

Please direct all future correspondence to me at the address above [or: to my new representative under the enclosed Form 2848].

[Signature, date]

If you do not have a copy of the original form, write a letter stating that you are revoking all powers of attorney on file for the listed periods, and sign it.
