Use this model letter

Copy or download the blank model letter, then replace every [bracketed field] and remove alternatives that do not apply. Read the guidance below before using it. These are templates, not official IRS forms; nothing is filed or sent for you.

Downloads work without JavaScript. Copy letter becomes available when the page is interactive.

Word is editable; PDF is for reading or printing. No account or personal information is needed.

When to use this

Use this model after an IRS call in which you and an employee discussed a specific agreement, action or follow-up. It is most useful when you need a clear written record of who said what and which tasks each side understood would happen next.

Work from notes made during the call. Include the date, approximate time, employee identification, tax periods, exact action discussed and your own promised documents or payments. Remove alternatives that were not discussed and avoid describing a tentative proposal as an approved agreement.

Sending the letter does not itself establish that the IRS approved or implemented the arrangement. Check your account and any written response, keep delivery records, and add the next follow-up to your IRS communication log.

Model letter and instructions

Send after any call in which something was agreed. Three sentences. It protects you if the agreement is never entered.
